The Hardship Assistance program was designed for the Thrive SMP community seeking assistance with basic needs due to situational and financial need. Assistance may be considered for Thrive SMP employees and the employee's immediate family members. Please see the program application for additional details.Â
The Thrive SMP committee members meet on a weekly basis to review applications. To ensure your request is reviewed promptly, please read and follow the instructions below:
Review guidelines and instructions prior to completing and submitting a request.
Download the Hardship Application. Complete all lines, as missing information may result in either a delay or denial of the grant request.
Attach any bills, receipts or other supporting documentation.
Sign and date the form.
Return the completed application to the Thriving Lives Coordinator via email, to the Thriving Lives lockbox, or you may deliver it to the Human Resources office (whose only function is to forward your application to the Thriving Lives Coordinator).
